You have been asked to complete Home Blood Pressure Readings. You can download the form from this page to record your readings.
You should record your blood pressure twice in the morning, and twice in the evening for 7 days.
You should then return the form to reception who will forward to your Doctor for review.
You can return by:
- Handing the form into reception
- Taking a scan or photo of the form and sending as an attachment on an online message
- Contacting reception via an online consultation and asking for a link to send a scan/photo of the form to us
- Completing an online consultation administrative request and typing in the results
