Registering with the Practice
Practice Area
Patients who live in the LS16 postcode and patients who live in the LS18 postcode above the A6120 ring road can register with the practice.
How to register
You can register by simply calling in at the practice with your NHS card and completing an application form at reception. Alternatively you can print off the form online, fill a copy in for each person and then hand it into reception. It would be helpful if you could also fill in a new patient medical form for each person as well. New patients will normally be offered a preliminary health check.
Overseas Visitors
You may be charged for seeing the doctor or nurse if you are visiting from abroad. For further information contact reception.
Updating Your Details
Please let us know if you move or change telephone number so we can update your records. You can change your details online if you wish. If you move out of the practice area it will be necessary for you to find another doctor.
Confidentiality
All patients' records on file or computer are completely confidential. The practice complies with the Access to Medical Records Act, the Data Protection Act and the Freedom of Information Act. Access to patient records is limited to health professionals and administration staff who are bound by a code of confidentiality. In order to maintain confidentiality, test results will only be given to the patients themselves or parents of minors.
