Repeat Prescriptions

How To Order Your Repeat Prescription

Repeat prescriptions can be requested once you are on regular treatment. Please allow 48 hours before collecting from reception. Please try to avoid the telephone as this could block the line for urgent calls. A prescription can be ordered in the following ways:

By email by filling in the online prescription request form.

By registering for SystmOnline and using the repeat prescription ordering service.

By fax on 0113 2953602

To maintain confidentiality, you need only provide:

  • your NHS number (shown on the top of the right hand side of your last prescription),
  • the name, strength and quantity of the medications required (e.g. frusemide tablets, 20mg, 56),
  • please also tell us if you have requested a chemist to collect the prescription from the surgery on your behalf.

In person:

It will help us to provide a better service if you return the right half of your previous prescription, ticking the boxes for the required medications. Please put in the repeat prescription box on the reception desk.

By post:

Please return the right half of your repeat prescription, or write clearly your name and the medications needed. Please include S.A.E. for return by post.

Medicines Waste

Unused prescription medicines cost Leeds PCT over £5 million every year. Please only order what you need.

Medication Review Dates

If your medication is due for review please make an appointment with the doctor. This is important as the doctor needs to monitor your treatment regularly. This may involve routine blood tests or blood pressure measurement.
