Certificates, examinations etc.
A fee may be payable for certain services not covered by the National Health Service. These include private health certificates, insurance claim forms, signing passports, and some travel vaccinations. For more information please talk to reception.
Chaperone
Please note if you are having an intimate examination a chaperone can be provided. This service is available to both male and female patients. Please discuss this with your doctor.
Dentistry
Unfortunately we are unable to help if you have a dental problem. If you are looking for urgent help from a NHS dentist call the Leeds Dental Advice Line on 0800 298 5787. If this is closed then call NHS Direct on 0845 46 47.
Disabled Access
The building was designed with disabled access as a priority and all consulting and treatment rooms are on the ground floor.
Privacy
Our reception is an open counter, but if you need to talk to the receptionist confidentially, she can arrange this for you.
Referrals
Once the doctor refers you to the hospital you can find out about how the referral is progressing by contacting the Referral Management Service (RMS) on 0113 305 7385. You can also contact the Patient Advice and Liaison Service (PALS) for information about hospital appointments, waiting lists and investigation results. They can be contacted on 0800 0525270.
Test Results
If possible please telephone the surgery on 2953600 after 2.00pm Monday to Friday. Please note that under no circumstances will any test results be given to anyone other than the patient themselves (or to the parents or guardians of young children).
Training
The practice is committed to the further training of health professionals. These include medical students, district and practice nurses, midwives and health visitors. At various times, qualified nurses will be training alongside practice staff in order to gain additional experience.
Violence
The practice operates a zero tolerance policy against aggressive and abusive behaviour. We shall not hesitate to call the police if necessary.
